Registered Nurse (RN) to join our team as Nurse Trainer and ensure
the highest quality services are provided to our clients.Since

Trainer to provide healthcare coordination and direct intervention,
as needed, for our adult residential programs. Additionally, the
Nurse Trainer will train and oversee Direct Support Professionals
in medication administration and supporting the healthcare needs of

bonusesAdvancement opportunitiesRESPONSIBILITIES:Provide healthcare
and wellness coordination for individuals in accordance with all
state and federal regulations, including but not limited to
HeM-1200s.Function as liaison and advocate for individuals'
healthcare, wellbeing, and inclusion with all members of individual
medical and interdisciplinary teams.Support individuals in
obtaining health services appropriate for their preventative,
acute, and/or chronic illness needs.Oversee that the health status
and physical conditions of individuals are assessed, monitored,
reported, and responded to in a timely and appropriate
manner.Participate in and attend assigned medical individuals'
appointments (as directed), individual specific, nurse,
departmental, and other meetings as required.Respond to emergency
medical situations in a timely manner, as appropriate.Train
unlicensed staff to meet the healthcare needs of individuals
served- including but not limited to medication administration,
delegated tasks, treatment protocols, and monitoring.Implement and
train employees on new and revised health related policies and
procedures in a timely manner as directed.Work with Program
Manager(s) in overseeing pharmacy and medication needs. This may
include but is not limited to reviews, obtaining medications, and
prior authorizations.Order medical supplies from established
formularies as applicable for your program.Maintain health-related
records, complete documentation and assessments per company policy,
federal, and state regulations, as applicable.Advise on potential
new admissions in regard to healthcare needs and appropriateness
for the program.Develop and update individual specific health risk
plans to ensure appropriate medical care for assigned
individuals.Manage transitions of care to and from hospital,
nursing home, rehab facility and/or another agency.Provide nursing
on-call per program requirements.Mandatory reporter of any
suspected abuse, neglect, or exploitation.KNOWLEDGE, SKILLS, AND
ABILITIES:Ability to express and exchange ideas through verbal and
non-verbal communication with all internal and external
stakeholders.Ability to effectively understand regulatory
requirements, multi-task, train staff, and provide clinical
oversight, as applicable.Possess the skills to operate
independently and/or in a team environment.Be in good standing and
able to delegate nursing services.QUALIFICATIONS:RN with current
New Hampshire license.Experience in community/long term nursing is
preferred.Experience working with individuals with intellectual,
developmental disabilities and/or mental illness.Valid Driver's
