Shared Living Coordinator - Merrimack NHMonday-Friday 8:30am-4:30pm or 9:00am-5:00pm $20.00/HRSome Essential Functions of the job include:

  • Leading the Aspire residential team to provide excellent services;
  • Participating on individual specific teams;
  • Interviewing, screening and contracting with quality Home Care Providers;
  • Overseeing the supports ensuring the health, safety and wellbeing of individuals' served including Medication Administration;
  • Mentoring and supporting Home Care Providers;
  • Ensuring regulatory and agency requirements are met;
  • Ensuring that all providers and staff are trained and certifications are up-to-date;
  • Partnering with Aspire clinician to ensure all behavioral/clinical needs are being addressed;
  • Analyzing behavioral data and evaluate the effectiveness of behavior treatment plans;
  • Providing financial oversight of individuals' funds;
  • Working with Aspire team to develop a program of inclusion activities that are consistent with the likes and abilities of each individual;
  • Ensuring the growth, independence, autonomy and safety of each individual via ISP and informal life planning goals;
  • Maintaining professional, positive, open dialogue and relationships with all internal and external contacts;
  • Participating in internal and external committees and teams
  • Participating in on call duties as required;KNOWLEDGE, SKILLS, ABILITIESMust demonstrate strong commitment to the Human Services field. Must have excellent interpersonal skills, verbal and written communication skills.CREDENTIALS - EXPERIENCE
    Bachelor's Degree or equivalent, related experience in discipline related to the delivery of supports to individuals with developmental disabilities.Competitive benefits package that includes:
