Job Description:

32 hours / week (8:00am - 4:30pm) + POSITION SUMMARY + Provides social work services, including crisis intervention and supportive counseling to clients/patients of all ages and their families relative to illness or injury. Assists clients/patients and their families in collaboration with multidisciplinary team, to develop safe and appropriate plans of care. + KEY RESPONSIBILITIES + Provides social work intervention to active clients/patients and family members. + Assesses clients'/patients' psychosocial, environmental, financial and functional status in order to coordinate appropriate plans. + Plans and implements patient care in collaboration with the physician. + Documents social work interventions in the client's/patient's medical record pertaining to the referral source, reason for referral, issues, recommendations and a treatment plan resulting in an established outcome. + Communications appropriate information to patients, peers, managers, physicians, department and other agencies involved. + Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes. + Completes documentation in clinical records, time sheets and quality control logs. + Adheres to VNA safety, organizational and ethical standards. + Adheres to VNA policies, protocols and procedures, including infection control standards. + Ensures confidentiality of employee, legal, client/patient, budget and all company matters. + Assists client/patient and family with difficulty in coping with residual physical disabilities and fear related to helplessness and death through clinical social work intervention. Serves as a coordinator for family meetings as needed. + Permits privacy for interviews and counseling as needed. + Acts as a resource to others (supervises student interns, peers, and other departments) in provision of client/patient care. + Case conferences weekly with BSW for all new BSW admissions and reviews/determines appropriateness, changes needed in developing a plan of care. Discusses with BSW any changes needed to patients plan of care for those patients followed by BSW. Documents case conference. + Functions effectively as a member of a team. + Demonstrates commitment to customer service. + Positively reflects the agency within VNA, the organization and the community. + When assigned, acts as Hospice Social Services Coordinator to coordinate the delivery of hospice social services as otherwise outlined above; participates in the development of the care plan; and works in conjunction with the Director of Patient Services to coordinate all social services required by the care plan and ensure their delivery. + REQUIRED KNOWLEDGE & SKILLS + Ability to work with frequent interruptions and respond appropriately to unexpected situations. + Familiarity with continuous quality improvement strategies. + Ability to travel to client/patient homes, meetings and conferences, which are held off-site. + Ability to plan and perform a wide variety of duties requiring general knowledge of policies and procedures applicable within area. + Ability to use good judgement. + Ability to receive training and guidelines from supervisor before assuming program responsibility and function independently following training. + Ability to communicate effectively and establish a cooperative, collaborative working environment. + Ability to accept constructive feedback and initiate appropriate actions to correct situations. + Ability to maintain good communications; ability to establish and maintain positive working relationships with employees, physicians, patients, families, and the public. + Ability to advance basic computer skills as the health system adds new systems or performs upgrades to existing systems. + EDUCATION/EXPERIENCE/LICENSURE + Education: Master of Science in Social Work required. + Experience: One (1) to three (3) years of relevant professional experience in a health care setting. + Certification/Licensure: None + OTHER REQUIREMENTS + Schedule: Generally works Monday through Friday, day shift; occasional requirements to cover weekends, special events, and/or emergencies after hours. + Other: Valid Driver's License is required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of auto mobile insurance required.Motor vehicle background report will be conducted.
