Job Description:
Encompass Health is seeking a dedicated LPN / LVN to join our
expanding team of healthcare professionals. Our ideal candidate
will provide compassionate, top-quality care and support in both
facility-based and home-based settings across our network.
Specializing in post-acute services, we pride ourselves on
fostering a supportive work environment aimed at ensuring
high-quality outcomes for our patients while advancing your nursing
skills in a dynamic healthcare setting. Provide nursing care to
patients in facility-based and home-based settings, adhering to
established guidelines. Monitor patient health status and respond
to their needs in a timely manner. Administer medications and
treatments as prescribed by the healthcare provider. Document
patient care services by charting in patient and department
records. Educate patients and their families on proper home health
care strategies and procedures. Coordinate with healthcare team
members to assess, plan, implement, and evaluate patient care
plans. Comply with safety standards and protocols, and participate
in facility health and safety training drills. Strong practical
nursing skills and knowledge. Excellent patient care and
communication abilities. Ability to work independently and as part
of a team. Proficiency in managing multiple tasks in a healthcare
setting. Experience with electronic medical records (EMR) systems.
Medical Insurance Dental Insurance Vision Insurance 401k Current
LPN or LVN license valid in the state of employment. CPR
certification. Additional certification in specialized care related
to post-acute services is desirable but not required.
