(Contract) Location: Woodsville, NH 03785 Shift: 5x8 Day Shift
Duration: 13 Weeks Pay Rate: $58 – $62 per hour Position Type:
Local or Travel Job Summary "Navitas Healthcare, LLC" is seeking an
experienced Physical Therapist to provide home care services. The
PT will evaluate and treat patients in their homes to improve
mobility, strength, balance, and overall independence.
Responsibilities Perform home-based physical therapy evaluations
Develop and implement individualized treatment plans Provide
therapeutic exercises, gait and balance training Assess home safety
and recommend equipment/modifications Educate patients and
caregivers on exercises and fall prevention Monitor progress and
update care plans as needed Document all visits and treatments in
EMR Collaborate with interdisciplinary healthcare team Requirements
Minimum 2 years of Physical Therapist experience Previous home care
experience required Active New Hampshire PT License BLS (AHA)
Certification Bachelor’s Degree in Physical Therapy (or higher)
Valid driver’s license and reliable transportation Ability to work
