Primary Care Nurse Practitioner to see geriatric patients in
nursing home facilities located in Portsmouth and Rochester, NH.
The individual will provide comprehensive nursing and medical care
to patients, adhere to organizational standards and policies,
demonstrate adequate knowledge, judgment, and technical skills to
ensure care plans and facility procedures are met. Requirements:
Valid NP license in the state of NH Board Certification Long-term
care experience- preferred Responsibilities will include: Managing
all Primary Care related consults History and physical exam
Medication review Prescribe medication and drugs Check in with
patients who have acute issues and establish a follow-up plan Reach
an informed diagnosis based on medical knowledge and the patient's
