Time, Days Locations: Claremont, NH area White River Junction, VT

RN Case Managers at the VNH screen, evaluate, re-evaluate, and
treat patients as well as coordinate and communicate treatment
activities with our interdisciplinary team members and the patient.
The RN Case Manager: Develops a care plan in collaboration with the
patient's physician, the patient, and their family, or the primary
caregivers upon admission. Periodically reviews the patient care
plan and recertifies the patient care plan as appropriate.
Coordinate the patient's care plan through interdisciplinary
communication and case conferences, as appropriate. Advise other
disciplines of discharges Minimum Qualifications: Must be a
licensed Registered Nurse in Vermont and New Hampshire or hold a
compact license. Graduate of an accredited school of nursing.
Medical/Surgical nursing and/or home health care experience. We
