seeking talented and dedicated Licensed Practical Nurses (LPNs) to

for our patients and residents.

Responsibilities of the Licensed Practical Nurse (LPN):
Consult and coordinate with interdisciplinary care team, which
includes medical providers and other healthcare professionals to
assess, plan, implement and evaluate individualized patient and
resident plans of care.
Participate in completing baseline and periodic assessments of
individualized care needs to ensure the most appropriate care needs
are met for each patient and resident.
Participate in resident care, to include accurately and safely
administering medications that are ordered by our medical
providers, and monitoring to ensure efficacy of medications
provided.
Provide nursing services to short term patients and long-term
residents within scope of practice, organizational policies, and
professional standards of care.
Requirements of the Licensed Practical Nurse (LPN):
Current license from state
Current/active CPR certification, preferred.
Effective verbal and written English communication skills.
Strong attention to detail and accuracy, excellent organizational
skills with ability to prioritize, coordinate and simultaneously
maintain multiple projects with high level of quality and
productivity.
Able to stand and/or walk for more than four (4) hours per day.
Able to bend and/or stoop for more than one (1) hour at a time.
