Job Description:
POSITION SUMMARY
To provide nursing care, education, and support to women with
breast cancer and breast health concerns in partnership with other
health care professionals. The incumbent collaborates with others
to create systems of care which facilitate education, diagnosis,
treatment and support for the patient, family and the
community.
PRINCIPAL DUTIES AND RESPONSIBILITES
Under the direct supervision of the Senior RN, Breast Health
Navigator and Manager, Breast Care Center and within established
Catholic Medical Center and departmental policies and procedures,
the incumbent performs the following functions:
* Collaborates with physicians and other health care providers in
planning individualized care for individuals with breast cancer and
breast health concerns, providing a link between them and
interdisciplinary service where care is delivered.
* Demonstrates effective interpersonal and communication
skills.
* Participates in the development, implementation and evaluation of
support and education programs for individuals with breast cancer
and their families.
* Accesses physical and psychosocial health status of the patient
and works with the provider to implement a therapeutic plan of
care.
* Consistently evaluates and documents the outcome of nursing
intervention and patient/family response to care; recognizes subtle
changes or trends in patient status; responds to unforeseen
situations in a timely, efficient and effective manner.
* Accesses individual and community educational needs. Maintains
close, collaborative relationships with community agencies
providing information, care and support to individuals with breast
care and breast health concerns
* Provides education, support, and coordination to assist patients
in securing appointments.
* Provides educational resources on breast health, breast cancer
and breast care. Connects patients and families to resources and
support services.
* Coordinates services throughout the continuum of Breast Care and
considers factors related to safety, effectiveness and cost in
planning for and delivery of patient care.
* Serves as a resource to others to provide direction and assist,
as necessary, in the performance of duties appropriately delegated
in accordance with the New Hampshire Nurses Practice Act and the
professional standards of nursing practice.
* Reinforces physician and patient relationships by promoting
communication between patient and healthcare providers.
* Provides support of services that include, but are not limited to
the following; assisting patient and family members with adjusting
a breast cancer diagnosis and treatment. Nutritional counseling,
palliative care, support groups, transportation services, and other
complimentary services. Psychosocial distress screening and
support.
* Assist department in developing and/or refining their educational
and support resources related to breast health.
* Take initiative to establish priorities, manage resources and
assist in implementing quality improvement initiatives.
* Assists provider with patient examinations and procedures,
including (but not limited to) surgical procedures, exams,
diagnostic studies and treatments.
* Monitors patients during and following procedures; looks for
reactions and other visible complications; react to emergency
situations; notifies the appropriate person in the event of an
emergency.
* Applies and removes dressings under the direction of the
provider.
* Serves on Catholic Medical Center Cancer committees and community
boards.
* Maintains certifications, accreditations and licensure.
* Performs similar or related duties as assigned or directed.
KNOWLEDGE, SKILLS AND ABILITIES
Individuals must possess these knowledge's, skills and abilities or
be able to explain and demonstrate that the individual can perform
the essential functions of the job, with or without reasonable
accommodation, using some other combination of skills and abilities
and to possess the necessary physical requirements with or without
the aid of mechanical devices to safely perform the essential
functions of the job.
* Physical requirements include ability to extend hand(s) and
arm(s) in any direction; stand for sustained periods of time; move
about on foot to accomplish tasks; raise objects from a lower to a
higher position or move objects horizontally from position to
position; pick, pinch, type or otherwise work with fingers.
* Ability to express or exchange ideas by means of the spoken word.
Essential functions include activities in which incumbent must
convey detailed or important spoken instructions to patients,
physicians, families and other employees.
* Knowledge of department and hospital confidentiality policies
pertinent to patient information and hospital data.
* Ability to receive detailed information through oral
communication and to make fine discriminations in sound.
* Ability to exert up to 20 pounds of force occasionally,
frequently, or constantly to lift, carry. Push, pull or otherwise
move objects.)
* Visual acuity sufficient for work which deals largely with
preparing and analyzing data and figures, computer terminal,
extensive reading, visual inspection involving small defects and
small parts.
* The ability to plan and perform complex work which involves new
or constantly changing problems. Involves participation in the
formulation and carrying out of policies, objectives and programs
for major divisions or functions. Considerable ingenuity and
exceptional judgment required to deal with factors not easily
evaluated. Interpret results and make decisions which there is
little precedent. Direct and coordinate the work of subordinate
supervision in order to attain objectives.
* Ability to endure periods of heavy workload or excessive
stress.
* Ability to work with frequent interruptions and respond
appropriately to unexpected situations.
* Ability to travel to meetings and conferences which are held
off-site.
* Demonstrated skills in empathetic listening, communication,
patient teaching, public speaking and working with groups.
* Ability to communicate well with others and maintain sensitivity
to patient concerns while working with other members of the Health
Care Team, including nurses, physicians, etc., and to provide the
best in comfort, caring and curing.
* Maintains current knowledge of breast cancer diagnosis, treatment
and survivorship.
WORK SCHEDULE:
* Generally Monday through Friday, day shift; Must be willing to
travel to physician offices, community agency sites, and to
programs and conferences. Involves occasional early morning
meetings (7:00am) and late evening group/support meetings, as well
as occasional weekend events (Health Fairs etc).
QUALIFICATIONS:
Education:
* Bachelor's Degree in Nursing (BSN) preferred or equivalent
nursing experience.
Experience:
* Three to five years' experience in nursing with a minimum of two
years in oncology.
Licensure/Certification:
* Current registration in the State of NH as a Registered
Nurse.
* Basic Life Support (BLS) certification required.
* Oncology Certified Nurse (OCN) certification desirable.
* National Consortium of Breast Centers (NCBC) Breast Patient
Navigation Certificate required.
* Breast Health Navigator Certification through EduCare
preferred.
