SummaryUnder the general supervision of the Director, Medical Home Navigation Program, the Medical HomeSocial Worker provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues.EducationMaster's Degree in Social Work required.Certification, Registration & LicensureLICSW preferred.ExperienceMinimum of three years clinical social work in a hospital, skilled nursing facility, home health or hospice, and/or community setting preferred. Demonstrate effective interpersonal, organizational and motivational skills. Demonstrate ability to function as a team player.
