  • POSITION SUMMARY
    • Please note: This job description is designed to be all inclusive for the functions of LNA, Unit Secretary, Phlebotomy, Telemetry, Radiation Therapy, and Telesitter. Depending on the area in which you work in, all sections of this job description may not pertain to you. Your manager will identify which areas apply to your position. The Licensed Nursing Assistant II, under the general direction of the Clinical Nurse Manager and the direct supervision of the Resource Nurse/Registered Nurse performs two or more of the functions listed below.
    • Licensed Nursing Assistant Functions as an integral member of the care delivery team by providing direct and indirect patient care under the direction of a RN and within the scope of practice of a Licensed Nursing Assistant (LNA). Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of an RN.PhlebotomyPerforms routine and emergency phlebotomy. Unit Secretary Under the supervision of an RN, performs a variety of clerical duties on patient units, including filing, transcribing, charting, sorting patient information, acting as receptionist, receiving and directing visitors, and answering the phone, etc.Telemetry Technician Provides indirect care in the department setting. Monitors cardiac rhythms, blood pressure and SPO2, via direct cardiac screen. Meets communication needs of the entire department. Maintains regulatory requirements, policies, procedures, and guidelines, and participates in performance improvement activities.Radiation TherapyFunctions as an integral member of the patient care delivery team by performing a variety of direct and indirect patient care activities, tasks and functions which support the delivery of optimal patient care under the general direction of the Director, Oncology Services and the direct daily supervision of the Clinical Leader/Registered Nurse and within established Hospital and departmental policies and procedures. Independently responsible for the coordination of the department s Follow up Clinic. TelesitterProvides continuous observation and surveillance of assigned patients. First line of action is to verbally redirect the patient from engaging in at risk behaviors. Summons the nursing staff if the patient requires assistance. Knowledgeable and supportive of patient and institutional confidentiality and related hospital policies.
    • KEY RESPONSIBILITIES
      • Licensed Nursing Assistant
        • Performs or assists patients in the completion of activities of daily living.
        • Provides basic support to patients and families in response to identified psychosocial needs and cultural and religious preferences
        • Performs clinical tasks within the scope of LNA practice, in accordance with policies and procedures of the Elliot Health System.
        • Reports all observations, symptoms, deviations from normal, and changes in patient s condition to the Registered Nurse.
        • Documents in selected areas of the medical record, as appropriate, according to policies and procedures and scope of practice.
        • Follows all infection prevention/universal precautions policies as appropriate.
        • Incorporates safety principles, policies and procedures into daily practice.
        • Ensures confidentiality of all patient, employee, legal, budget, and company matters. Maintains patient privacy and dignity.
        • Phlebotomy
          • Performs routine and emergency phlebotomy.
          • Telemetry
            • Completes rhythm analysis and interpretation and posts rhythm strips at the specified times.
            • Communicates information regarding rhythm changes and vital sign abnormalities to the primary nurse and charge nurse.
            • Demonstrates knowledge of location of patients that are off the unit for testing.
            • Documents in selected areas of the medical record as appropriate according to guidelines.
            • Maintains telemetry alarm notification log.
            • Ensures that communication devices are working properly, assigned and distributed to nursing staff.
            • Unit Secretary
              • Contributes to ensuring effective and efficient communication with visitors, callers, physicians, staff from other departments and others as appropriate.
              • Meets and greets visitors to the department; determines the purpose of visit and responds or refers to appropriate staff member.
              • Updates and maintains patient assignment board, Census Change Sheets, and all other patient lists.
              • Supports effective and efficient patient care processes through timely, accurate transcription and entry of orders/data.
              • Maintains Unit Secretary task list to ensure patient care which are verified by the registered nurse, to appropriate patient care documents.
              • Assembles patient charts for all admissions to the unit to include all appropriate documents
              • Prepares patient charts for patient transfers by copying and organizing required documents according to specific payer criteria.
              • Disassembles and reorganizes charts upon discharge before forwarding to medical records.
              • Coordinates completion of patient death certificates by contacting appropriate parties.
              • Supports the smooth functioning of the unit by performing a variety of clerical and coordinating activities as designated by the unit.
              • Updates and maintains the daily communication device list for the unit; ensures that all RNs receive communication device at the beginning of the shift and return them at the end of the shift.
              • Radiation Therapy
                • Schedules, tracks and assures timeliness of diagnostic testing. Obtaining insurance authorization for diagnostic testing.
                • Schedules all follow up clinic appointments.
                • Reviews patient s charts to ensure all necessary information is obtained prior to patient visit.
                • Obtains an updated medication list for all follow-up patients. Coordinates needed services/referrals for follow up patients.
                • Tracks patients missed appointments, and notification to patient to reschedule and sending a certified notice for repeated missed appointments, per protocol.
                • Assists physicians with patient physical exams/procedures/preparation of needed equipment.
                • Ensures all exam rooms are adequately stocked and cleaned. Checking for and replacement of any expired items. Responsible for lined inventory and distribution within the department.
                • Coordinates weekly treatment visits for physicians and RNs.
                • Orders all departmental needed clinical supplies in the Lawson system. Tracks supply of bulk stores and MDS items and makes recommendations to adjust par levels.
                • Telesitter
                  • Maintains constant visual observation of the patients receiving remote visual monitoring.
                  • Participates in the hand-off, during shift change or when admitting new patients to the system, of pertinent information/behavior about patients being monitored.
                  • Ensures another telesitter in place before leaving monitoring station.
                  • Verbally redirects patients when necessary.
                  • Provides patients with explanations as necessary, but does not counsel or provide opinions.
                  • Seeks help or advice as soon as possible when a patient appears to pose a threat to themselves or others. Participates in collaborative identification and reporting of patient safety issues.
                  • REQUIRED KNOWLEDGE & SKILLS
                    • Licensed Nursing Assistant
                      • Demonstrates and applies effective communication skills: verbal, written, visual and listening.

                      • Telemetry Technician
                        • Demonstrates the knowledge and skills necessary to complete the duties of the position that are identified on the competency assessment checklist.
                        • Knowledge of cardiac rhythms and ability to recognize normal and abnormal rhythms.
                        • Demonstrates good communication and interpersonal relations including the ability to appropriately and courteously relate to others.
                        • Able to utilize electronic technology.
                        • Unit Secretary
                          • Strong organizational skills.
                          • Radiation Therapy
                            • Knowledge/proficiency of three Electronic Medical Record Systems: EPIC, ARIA and Mosaic and Lawson ordering system.
                            • Ability to communicate with insurance companies for the authorization process which requires knowledge of clinical aspects of the patient.
                            • Telesitter
                              • Demonstrates the knowledge and skills necessary to complete the duties of the position that are identified on the competency assessment checklist.
                              • Demonstrates good communication and interpersonal relations including the ability to appropriately and courteously relate to others. Able to communicate effectively with patients through the headset and microphone.
                              • Ability to use discretion and courtesy when interacting with patients, visitors, and staff.
                              • Ability to utilize the remote visual monitoring system with visualization of up to 12 patients at a time and remain alert at all times when on duty.
                              • Ability to multitask and prioritize simultaneous situations.
                              • EDUCATION/EXPERIENCE/LICENSURE
                                • Education: High school diploma or equivalent required.
                                • Experience: None required.
                                • Licensure/Certification: Current Nursing Assistant License in the State of New Hampshire. BLS certification required. Health Unit Coordinator (HUC) certification preferred.
                                • Software/ Hardware: MS Office
                                • OTHER REQUIREMENTS
                                  • Schedule: Variable, determined per unit.
