Job Description:
Catholic Medical Center is looking to hire a Director of Case
Management and Population Health to join their team! Offering

The Director of Case Management and Population Health will oversee
the operations and staff for Case Management, Social Services,
off-site Care Coordinators and Utilization Review. Responsibilities
include, but not limited to, directing the work of the Case
Management department in accordance with federal, state and local
guidelines and regulations; facilitates appropriate utilization of
hospital and community resources to provide high quality,
cost-efficient care, and achieve throughput, length of stay, and
clinical quality goals; builds and maintains strong working
relationships with post-acute partners to ensure successful
transitions of care and positive patient outcomes; reviews
re-admissions and oversees opportunities to reduce preventable
re-admissions; presents reports and statistics to the Utilization
Review; responsible for the departments budget, contracts with
external vendors, and applicable policies/ standard operating
procedures/ work instructions; plans and implements recruitment and
retention strategies for areas of responsibility in collaboration
with Human Resources.
The Director is responsible for providing leadership, direction and
support in the development, integration, management and growth of
Medicare Advanced Payment Models - which may include Accountable
Care Organization (ACO, Bundled Payments, and/or other future
models of Medicare reimbursement - as well as value-based contracts
with commercial insurers. Responsibilities include network
development, liaison in managing network contracting and payer
relations, finance, budget management, advanced analytics, member
services and legal/regulatory compliance. In close partnership with
the Associate Chief Medical Officer , the Director will be
responsible for setting a strategic vision for Care Management and
overseeing day-to-day operations of the program. He/she will work
with the Clinical Population Health Analyst to maintain close
monitoring of outcomes and performance data, and the Clinical
Resource specialist to ensuring adjustment to program design and
implementation to achieve program goals, In addition the Director
will be accountable for helping to drive quality outcomes.
Education: Bachelor's Degree in nursing required (Masters degree in
Nursing or Business preferred.)
Experience: Minimum of five years successful administrative
experience in a hospital Case Management Department. Experience in
program development is required.
