Job Description:
Overview: Military Veterans who access Veterans Administrative
health care, and have daily care needs, and who transition to
community living have services available to them. Tempus Community
Services is an approved Area Disability Network Agency (ADNA) and
part of the team of services to support that Veteran through their
transition to home. Once home, there are also services to assist
that Veteran maintain their home and receive the care they need.
Veterans are referred to Tempus ADNA for Case Management Services,
from the VA. These Veterans can be transitioning home, or are
currently home and needing Case Management Support. Tempus is
looking for a Veteran who can assist Veterans self-direct and
empower them with good knowledge to manage their home and care
needs. The VDC Case Manager will support the Veteran through the
process of establishing needs and map out plans to address those
needs. The VDC CM will then monitor the program management to
ensure positive outcomes of established plans. Actively participate
as a member of the Resource department team and the larger
organization in support of business objectives. Essential Duties
Promote VDC Case Management services through a variety of manners,
working with Public Relations. Conduct Referral, Intake and
orientation, and ongoing Case Management services. Reference all
VDC rules and regulations to conduct business. Provide input for
updates to Tempus VDC CM documents, or processes. Process VDC
documents to electronic records storage according to regulations.
Provide video, phone and in person services as required by VDC
regulations. Provide monthly reports as required by VCD regulation,
to VDC VA Case Manager Work as part of a team to ensure the best
services to Veteran. Establish objective and critical thinking when
determining how best to address the Budget Plan of Care that is
approved by the VA. Support Veteran to identify needs, and
community resources to meet those needs. This is in keeping within
the approved VA budget plan. Support Veterans to keep open
communication with all program related matters. Participate in
community events that promote resources in our communities. Assist
Veterans to access their communities and have knowledge of
resources. Required Experience Background in providing community
services to people with disabilities. Background working with plans
of care, or service plans. Attention to detail required. Basic
computer skills in Microsoft Office/Outlook/Excel and the ability
to navigate database. Medical knowledge helpful. Professional
telephone manner, computer database skills, familiarity with office
procedures and equipment. Excellent, interpersonal, communication,
organization and writing skills. This position requires the utmost
cooperation in providing a positive environment to create a

funds management/monitoring Background or knowledge in
self-direction services. Strong interpersonal skills – telephone

confidentiality Perform all other duties as may be required.
Education: Advanced degree in Human Services or related years of
experience. Work Environment This job operates in a professional
office environment, a home office, or in the home of the Veteran.
There may also be times when a visit may need to happen in an
alternate location such as a facility. This role routinely uses
standard office equipment such as computers, cell phones, and uses
electronic signature processes for documents. Physical Demands The
physical demands described here are representative of those that

functions of this job. While performing the duties of this job, the
employee is regularly required to talk and/or hear. The employee is
frequently required to sit; stand; walk; use hands to finger,
handle or feel; and reach with hands and arms. Staff may climb and
descend stairs, or may need to step over items as to keep safe.
Travel Travel is required for this position. Must have a valid
